OF UNSATISFIED JUDGMENT – cause no. _______________

THIS
AGREEMENT, entered into between ____________________________, hereinafter
referred to as "JUDGMENT CREDITOR" and Financial Service Corporation, herein
after referred to as "ASSIGNEE". JUDGMENT CREDITOR hereby assigns to ASSIGNEE
all rights, title and interest in the judgment entered against
________________________ in the amount of

(court) plus interest of ______% from date of award to date of collection.
AS
CONSIDERATION for this assignment ASSIGNEE will pay JUDGMENT CREDITOR fifty
percent of all funds collected and pay all expenses required to satisfy the
judgment including asset search, skip tracing and collection. JUDGMENT CREDITOR
will incur no expenses in collecting the above mentioned judgment. ASSIGNEE
will retain any increase in the judgment amount to cover post-judgment costs of
collection awarded by the court
JUDGMENT
CREDITOR understands ASSIGNEE is not a collection agency.
THE ASSIGNEE
agrees to reassign said judgment back to the JUDGMENT CREDITOR upon receipt of a
written request, after one year he date of Assignment, in the event of
unsuccessful collection. During the active period of this ASSIGNMENT, the
ASSIGNEE has total and exclusive right to collect the subject judgment. If the
defendant's assets, personal property, real property or wages are liened/or
garnished before the expiration of this agreement, both parties agree that this
AGREEMENT will continue beyond the expiration date until such date as collection
via said liens/or garnishments are sufficient to satisfy the judgment.
